The present invention provides a modified conjugated diene-based polymer of which one end has amine and the other end has a nitrogen atom and a silyl group substituted with at least one alkoxy group, wherein the modified conjugated diene-based polymer is prepared by: synthesizing an initiator having an anionic end containing amine and a cationic end containing an alkali metal; using the initiator to polymerize a combination with an aromatic vinyl monomer, thereby synthesizing a polymer of which one end is modified; and coupling or reacting the polymer with a compound containing a nitrogen atom and at least one silyl group substituted with an alkoxy group. The polymer, when being mixed with silica as a reinforcing agent, can have excellent exothermicity and exhibit tensile strength, abrasion resistance, and resistance to a wet road.
