The invention relates to a method for operating an internal combustion engine (1) in which a blow-by stream (10) containing a fine oil mist is discharged from a crankcase (2), and supplied to the charge-air side of the internal combustion engine (1), by means of a gas recirculation device (11), wherein the blow-by stream (10) is treated as it passes through an oil separator (13) arranged in the gas recirculation device (11) and, before and/or during the treatment in the oil separator (13), a coarse oil mist (16) is moved into the blow-by stream (10). The invention also relates to an internal combustion engine (1) having a crankcase (2) which is connected via a gas outlet opening (9) to a gas recirculation device (11), and in which, in the crankcase (2), there are arranged guide elements (14) for generating and maintaining a gas circulation (15).
