<p>Computer system and method for transmitting healthcare messages to an automatically identified set of patients. The messages are intended to guide patients toward completion of recommended healthcare actions based on the individual patient's profile, as well as incorporating aggregate patient population data (e.g., generally accepted or current best practices) in one or more healthcare fields for driving the recommendations to the patient population most likely to benefit from such practices. In addition, the system/method allows different providers (e.g., healthcare providers, insurance providers, and other providers) to establish their own rules for their own subsets of patients. The system/method may include monitoring and tracking patient completion of the recommended actions.</p> |
