PROBLEM TO BE SOLVED: To provide an information generation device, an electronic pen system and a program capable of suitably digitizing medical recording forms.SOLUTION: A terminal apparatus 3 recognizes a position where an attached paper 6 is positioned in a chart form 5 by recognizing a position of an end point within an interrupted stroke based on a piece of recorded information received from an electronic pen 1. The terminal apparatus 3 recognizes that an inspection item specification column 21 is selected based on the recorded information received from the electronic pen 1, and acquires examination data of the inspection item corresponding to the selected inspection item specification column 21 from a server 7. With this, the terminal apparatus 3 generates an electronic data of the chart form 5 having the attached paper 6 and displays the electronic data on the display 30.
