According to an aspect of an embodiment, a method of delivering information-enabled personalized healthcare in a clinical, non-research setting may include capturing one or more data streams where each of the data streams relates to health care of a patient. The method may further include integrating the data streams to generate integrated diagnostic data and analyzing the integrated diagnostic data to generate analyzed diagnostic data. The method may further include curating the analyzed diagnostic data and generating an integrated report for presentation to a physician of the patient based on the curated analyzed diagnostic data.

1. A system for the electronic delivery of information in personalized healthcare, the system comprising:
a social network configured to facilitate interactions between multiple participants with respect to health care of a patient; a data analytics unit configured to receive information from one or more of the multiple participants concerning the health care of the patient, the data analytics center further configured to analyze the received information, the analysis including curating the received and analyzed information; and a reporting unit configured to generate an integrated report based on the analysis of the received and analyzed information and to deliver the report to one of the multiple participants.
