PROBLEM TO BE SOLVED: To provide a door opening and closing device for a vehicle which achieves a reduction in space for arranging a pair of pulleys for guiding a cable from a vehicle door to a vehicle body in a front-rear direction of the vehicle and a reduction in number of components. SOLUTION: The door opening and closing device for a vehicle comprises a rotating shaft 32 fixed at a side of a slide door and extending in a vehicle width direction; a first pulley 33 supported by the rotating shaft 32 so as to be rotatable thereabout and within which a first cable 24, which is reeled in and reeled out from a drum, is engaged in order to guide a terminal to be connected to the side of the vehicle body; and a second pulley 34 supported by the rotating shaft 32 so as to be freely rotatable thereabout and so as to be arranged in parallel with the first pulley 33 in the vehicle width direction and within which a second cable 25, which is reeled in and reeled out from the drum, is engaged in order to guide the terminal to be connected to the side of the vehicle body.
