A syringe (10) is provided for introducing into a patient a combination of adipose tissue and an additive, for example, a hydrogel additive. The syringe includes two cartridges (12,18) inserted into housing (26) in a side-by-side manner. A lever (514) for actuating the syringe and dispensing from both cartridges simultaneously acts together with toothed element(s) (516,518) as a ratchet mechanism (512) for advancing plungers (476,478). A mixing tip (30) at the distal end is responsible for mixing of the cartridge components prior to extrusion from a needle or cannula(38).
