PROBLEM TO BE SOLVED: To provide a delivery slip capable of making the generation of a curl on a receiving slip as small as possible even if the receiving slip is stuck so as to be releasable in a pseudo-adhering state on a sheet substrate is released, and preventing a reading error from being generated even if the mechanical reading processing of the collected receiving slip is performed. SOLUTION: The delivery slip includes: a sheet substrate which is directly stuck on a baggage by a pressure-sensitive adhesive layer formed on a back face; and the receiving slip which is provided at the upper side of the sheet substrate and laminated in a releasable state from the sheet substrate through the pseudo-adhering layer.
