a proximal portion including a pliable support and at least one securement arm integrally formed with the proximal portion and extending away from the pliable support, the proximal portion defining a slot extending through the proximal portion from a top surface of the proximal portion to a bottom surface of the proximal portion, the slot configured to receive at least a portion of a catheter, wherein the at least one securement arm is foldable over the portion of a catheter received in the slot to retain the portion of the catheter within the slot; and a distal portion including a first adhesive surface configured to be folded over the proximal portion to secure the proximal portion to a patient. |
