PROBLEM TO BE SOLVED: To provide a substrate for suspension capable of mounting an actuator element in which a pair of electrodes are easily and inexpensively formed.SOLUTION: A substrate 1 for suspension according to the present invention includes: a metal support layer 20; an insulating layer 10 disposed on the metal support layer 20; and a wiring layer 40 disposed on the insulating layer 10. The wiring layer 40 includes a first element connection terminal 45 connected to an actuator element 104. The first element connection terminal 45 includes a connection surface 45a connected to one electrode 104a of the actuator element 104 disposed on the side opposite to the side of the insulating layer 10. The metal support layer 20 includes: a tongue part 21 for supporting a head slider 112; and a second element connection terminal 22 extended upward from the tongue part 21, and connected to the other electrode 104b of the actuator element 104.
