An anchor assembly (110) is disclosed to be anchored in a concrete bore-hole. The anchor comprises an anchor bolt member (112) with an enlarged head portion (128) and an expansion sleeve member (114). A plurality of annular grooves (138) are disposed on a forward portion of the sleeve (114) while the rearward portion is non-grooved and solid. When the anchor bolt (112) is moved through the expansion sleeve (114) the enlarged head will expand the forward portion of the sleeve (114) so that the grooves are forced into engagement with the borehole. Subsequently the enlarged head will expand the rearward portion to achieve a maximum area and volume of interference with the borehole. The expansion sleeve (114) is tapered with a larger rear end to enhance pull-out resistance.
