Provided is a polyethylene-based resin forming sheet capable of being easily removed by washing foreign substances attached to glass. The provided polyethylene-based resin forming sheet: contains a polyethylene-based resin, a polymer type antistatic agent, an anionic surfactant, and a nonionic surfactant; contains the polymer type antistatic agent of 5-15 wt%, the anionic surfactant of 0.1-5 wt%, and the nonionic surfactant of 0.1-0.5 wt% about the polyethylene-based resin 100 wt%; and has a weight ratio of the anionic surfactant which is 1-10 wt% about the nonionic surfactant.
