The method for the surgical treatment of the advanced chemoresistant pulmonary tuberculosis comprises the surgical resection of the ribs V, IV, III, II and I in increasing extent in such a way that the straight line could be drawn from the edge of the rib V resected paravertebrally (8-) to the edge of the rib II resected parasternally. Following apicolysis and pneumolysis, at the apex of the lower lobe (C) the angle of scapula is resected by 4-, the free ends of the ribs V and IV are ligated, the thoracic cavity is closed and the soft tissues are sutured. Prior to the surgery, pneumoperitoneum is applied on the side of the pathological process for two months. The extent of pneumoperitoneum is adjusted depending on the mobility of diaphragm. Pneumoperitoneum is also maintained postoperatively under X-ray control. |
