The present invention relates to a forming apparatus for a freeform five-shaft polymeric scaffold comprising: a support table which is arranged on a base; a worktable which is arranged to be separated from the support table and allows a polymeric scaffold to be formed; a dispenser device which is arranged on an upper part of the support table and discharges raw materials forming the polymeric scaffold towards the worktable; a dispenser transfer means which transfers the dispenser device to an X axis, an Y axis and a Z axis; and a worktable transfer means which rotates the worktable from side to side and tilts the worktable back and forth. The worktable transfer means comprises: a pair of support brackets which are arranged on the base to be separated from each other with the worktable as the center; a rotation drive part which rotates the worktable with a vertical axis as the center formed by passing through the worktable by being combined to a lower region of the worktable; and a tilting drive part which tilts the worktable and the rotation drive part back and forth with a horizontal axis as the center by being combined to the support bracket in a transverse direction.
