PROBLEM TO BE SOLVED: To provide a delivery slip which can be partially divided before delivery and has good feed aptitude, and allows a person to find precisely who was recipient at a delivery destination.SOLUTION: The delivery slip comprises a surface base material 10a and a tack base material 40b adhered via a peeling layer 20 and an adhesive layer 30 and the surface base material 10a and the tack base material 40b are able to be peeled. A division line S is formed on the surface base material 10a in order to divide the surface base material 10a into a body part and a divided part 13. The tack base material 40b is adhered on a part overlapping to the body part of the surface base material 10a. A base material for adjustment 60 is adhered on a part overlapping to the divided part 13 of the surface base material. And an entry column K is formed on the surface base material 10a. The peeling layer 20 and the adhesive layer 30 are not formed on the parts of the peeling layer 20 and the adhesive layer 30 corresponding to the entry column K. And a color developing layer 10b is formed on the side where the surface base material 10a contacts with the peeling layer 20. And a developer layer 40a is formed on the side where the tack base material 40b contacts with the adhesive layer 30. |
