PROBLEM TO BE SOLVED: To align a patient position and an image position without using a medical image pick-up marker. SOLUTION: A medical image display device system includes: a medical image pickup device 13 configured to pick up a medical image of a subject 301; a display device 6; a subject position detector 311 configured to detect positional information of a subject; an image pick-up device position detector 312; a three-dimensional position measuring device 15 configured to measure positional information for the subject position detector and image pick-up device position detector; and a medical image display device 1 configured to align positional information for a subject and positional information for a medical image. The medical image display device calculates a simultaneous coordinate transformation matrix T<SB POS="POST">5</SB>for carrying out coordinate transformation via an image pick-up device coordinate system C<SB POS="POST">3</SB>, image pick-up space coordinate system C<SB POS="POST">4</SB>, and a three-dimensional position measuring coordinate system C<SB POS="POST">1</SB>, thereby using the medical image coordinate system C<SB POS="POST">5</SB>and subject position detection coordinate system C<SB POS="POST">2</SB>of DICOM information, marked on a medical image, as alignment information for the positional information of the subject and the positional information of the medical image.
