The device has a cup or bowl-shaped endoscope cap (1) fixed or formed at a distal end of a shaft-like insertion unit (2) i.e. endoscope. The cap includes an expansion sleeve (3). A resiliently prestressed tissue clip (4) is attached to the expansion sleeve. The tissue clip is removable by a thread (11) and a removal ring via a distal front edge of the cap. A cable loop (33) is arranged inside the expanding sleeve and is held on an inner wall of the expanding sleeve at predetermined axial distance to the distal front edge of the cap.
