A technique for intraabdominal plastic reconstruction of the giant postoperative ventral hernia comprises the excision of postoperative cicatrix, the separation and incision of the hernial sac, the separation of the adhesions between the abdominal organs, the greater omentum, and the edges of the hernial defect, the mobilization of the muscular-aponeurotic edges of the hernial defect from the subcutaneous base, the placement of the composite mesh implant and its fixation around the defect of the abdominal wall to the muscles and aponeurosis, and the suturing of the flaps tailored from the hernial sac. Then, the aponeurosis of the external oblique muscle is cut along the edge of the sheath of the abdominal rectus muscle on the right and on the left, the rectus muscles are mobilized from the oblique muscles eliminating the contracture of rectus muscles that are sewed without tension over the mesh implant. |
