A treatment assembly (20) has a probe (12) and a sheath (14). The sheath (14) has an inner tube (42) and an outer tube (46). The inner tube (42) has an opening (60) on the side surface. The assembly (20) is assembled in the following manner. The probe (12) is inserted into the inner tube (42). When doing so, the lengthwise axis (L) of the probe (12) is offset from the center axis (C) of the inner tube (42). An arc-shaped body (72) is inserted into the inner tube (42) via the opening (60) and engages with an arc-shaped concave part (34b) of the probe (12). When doing so, the lengthwise axis (L) matches the center axis (C). Then, the outer tube (46) covers the outside of the inner tube. When the assembly (20) is attached to a handle (16) on a treatment tool (10), an ultrasound transducer (18a) of the handle (16) transmits ultrasonic vibrations to the probe (12).
