An apparatus for generating an electronic form in a medical information system and an electronic form generation method thereof are provided. The apparatus includes: an MR item management unit for generating MR items and storing the MR items in an MR list; a form template management unit for generating a form template so as to include at least one MR item; a menu processing unit for providing an MR item generation menu and a form template generation menu and, when a user inputs a request for a form template, transmitting the requested form template to a user terminal and display the form template on a screen; and a form processing unit for matching examination information input from the user terminal to an MR item on the displayed form template to generate an electronic form, wherein the form processing unit matches examination information input from the user terminal to an MR item on the displayed form template to generate an electronic form, and the form template management unit matches arrangement information to an MR item included in the form template and stores the arrangement information.
