PROBLEM TO BE SOLVED: To provide a delivery slip by which it is possible to clearly understand who is an addressee at a destination.SOLUTION: A delivery slip includes a surface base material 10a and a tack base material 40b which adhere to each other through a peeling layer 20 and an adhesive layer 30. The tack base material 40b is stuck on a peeling base material 40d through a pressure sensitive adhesive layer 40c, and the surface base material 10a and the tack base material 40b can be separated. A print area K is formed on the surface base material 10a. The peeling layer 20 and adhesive layer 30 are not formed on portions of the peeling layer 20 and adhesive layer 30 corresponding to the print area K. A color former layer 10b is formed on the surface base material 10a on a side contacting with the peeling layer 20, and a developer layer 40a is formed on the tack base material 40b on a side contacting with the adhesive layer 30. |
