<p><P>PROBLEM TO BE SOLVED: To attain a system for promptly collecting information about the dwellers, etc., when a disaster occurs and to provide an image processor that constitutes the system. <P>SOLUTION: The image processor is provided with a registration form generating part 51 which selects the template of a form for information registration, in which items for registering information about the disaster are expressed in a list form, embeds information about registration items in the template, to generate the form for information registration and an image formation part which outputs the generated form for information registration. In addition, the image processor is provided with an entry information database 64 which holds the already registered information about the disaster.
