The present invention relates to an antibody-cytotoxin conjugate in which a site-specific cytotoxin is conjugated at an N-terminal amino group with respect to a cancer cell surface antigen and a preparation method thereof. In addition, the present invention relates to a composition for cancer diagnosis or treatment using the antibody-cytotoxin conjugate to diagnose or treat diseases under in vivo and in vitro conditions. Using the composition for cancer treatment including the antibody-cytotoxin conjugate enables specific targeting and relieves increased antibody-dependent cytotoxicity and side effects due to strong cytotoxicity, compared to independent treatments of antibody or cytotoxin. Furthermore, since the conjugate maintains homogeneity, the heterogeneity of the conventional antibody-cytotoxin conjugate can be overcome, and the present invention can be applied for developing a more effective antibody-cytotoxin conjugate since the cytotoxin is combined to the alpha-amine and N-terminal of the antibody using antibody that is not genetically manipulated.
