PROBLEM TO BE SOLVED: To provide an image reader easily determining where an optical defect such as dust is, and thereby improving maintenance operability, and also to provide an image forming apparatus equipped with the same. SOLUTION: The image reader 100 includes: a light source 211 comprising an array of a plurality of light emitting elements 212(1), ..., 212(n) irradiating a document G with light in a main scanning direction X; and a photoelectric converting element 205 receiving reflected light from the document G, wherein the plurality of light emitting elements 212(1), ..., 212(n) can be driven individually by the light emitting elements. The image reader determines a state of a defect image which is possibly generated owing to an optical defect C on an optical path LP1 or LP2 between the light source G and photoelectric converting element 205, specifies the defective position of the optical defect C in the main scanning direction X, based upon a determination result of the state of the defect image, and informs the specified defective position through light emission patterns of the plurality of light emitting elements 212(1), ..., 212(n).
