PURPOSE: A sheet and a recording medium with index foldable index are provided to perform an index function without a separate structure by including an index part in which a part of sheets is cut. CONSTITUTION: A sheet (10) comprises information indication domain (1) which records information and an index part (2) which is positioned at the edge. In the index part, an index part (3) in which a part is cut while being connected to the sheet is formed. The index part is folded to be protruded to outside of the sheet while being connected to the sheet. The index part is formed with materials different from the information indication domain. |
