An apparatus for providing percutaneous access to a vessel of a patient over a wire guide. The apparatus comprising an outer sleeve with open ends, and inner sleeve with distal open ends received in the outer sleeve, and a stiffening cannula within the lumen of the inner sleeve. The inner sleeve and the stiffening cannula being engaged such that relative axial movement between them is inhibited and the inner sleeve distal portion shields the stiffening cannula during percutaneous introduction into the vessel.
