PROBLEM TO BE SOLVED: To extract density unevenness distribution information showing a distribution of periodic density unevenness even when the periodic density unevenness including density unevenness which cannot be expressed only by sine waves or cosine waves occurs due to the rotation of a plurality of rotary bodies which image formation means has.SOLUTION: An image formation control device controls image formation means so as to form an extracting-image for extracting density unevenness distribution information showing a distribution of periodic density unevenness for each rotary body; extracts an average image obtained by averaging a plurality of periodic images which are included in read images obtained by reading extraction images formed by the above-mentioned control and correspond to the rotation cycles of object rotary bodies which are extraction objects of the density unevenness distribution information while synchronizing the phases of the object rotary bodies in the rotation direction, as the density unevenness distribution information showing the density unevenness due to rotation of the object bodies of rotation; generates a correction value for correcting the density unevenness expressed by the extracted density unevenness distribution information; and corrects image data or the exposure amount of exposure means supplied to the exposure means by using the correction value.
