PROBLEM TO BE SOLVED: To provide a device and method for evaluating an optical waveguide that is capable of efficiently evaluating optical characteristics of a multichannel optical waveguide.SOLUTION: An optical waveguide evaluation device 1 evaluates optical characteristics of an optical waveguide 9 including four core parts 94 and mirrors for optical path conversion (optical path conversion parts) 99 that are formed on respective one ends thereof. The optical waveguide evaluation device 1 comprises: a light emitting unit 3 for emitting light that is caused to be incident into the optical waveguide 9; a light receiving unit 4 for receiving light emitted from the optical waveguide 9; and a measurement part 5 for measuring intensity of light received by the light receiving unit 4. Further, the optical waveguide evaluation device 1 comprises: an optical waveguide type branch part 6 that is provided between the optical waveguide 9 and the light emitting unit 3 and that is configured so that a branch point 62 where one waveguide part 61 branches into two waveguide parts 61 is connected in two-step series; and an optical waveguide type confluence part 7 configured so that a confluence point 72 where two waveguide parts 71 join together for one waveguide part 71 is connected in tow-step series.
