PROBLEM TO BE SOLVED: To provide an image reading apparatus capable of quickly detecting a tilt of a document carrier, and to provide an image forming apparatus with the same. SOLUTION: Decision charts 77a and 77b are attached to both back and forth ends of a device body in a lower surface part 73a of a carrier guide 73 provided to a document carrier device 27 of the image reading apparatus 6. Upon detecting the tilt, first and second carriage 68 and 69 scan along a reading line Ls while being stopped under the decision charts 77a and 77b, and detect the tilt of the document carrier device 27 by detecting a displacement with a CCD sensor 67 between a straight line L1 connecting an intersection point A1 of the decision chart 77a and an intersection point A2 of the decision chart 77b, and a reading line Ls.
