The instrument (1) e.g. surgical pliers, has an end effector (8) fastened to a distal end part of a shaft pipe (2) and including branches (11) to be fastened to a mounting piece (9) connected with the end part by releasable couplers, where one of the couplers is fastened with a locker. The piece and the end part free-movably stay in rotatable positive engagement with the couplers in axis direction of the pipe. The locker comprises a union nut (7) that is screwed to a coupled piece and on which the mounting piece is rotated, where the nut is firmly supported in the axis direction.
