Provided are a method and system for assessing a condition of a patient with a chronic condition. The system includes a storage device that stores a plurality of variables relating to symptoms being experienced by the patient during an exacerbation of the chronic condition. A baseline component establishes a baseline indicative of a normal condition of the patient with the chronic condition while not experiencing an exacerbation. A risk assessment component is operable to compare the condition of the patient as determined based on the variables to the baseline to determine a severity of the exacerbation of the chronic condition relative to the baseline.
