A medical image compression device is provided with: an object-to-be-compressed acquisition unit (100) which acquires an image to be compressed which is a medical image to be compressed, and finding information regarding the image to be compressed; a disease range acquisition unit (130) which divides the image to be compressed into each disease area on the basis of the finding information; a pixel value prediction unit (190) which refers to a prediction knowledge database (180) holding, for each disease, frequency distribution of a pixel value of an arbitrary pixel corresponding to pixel values contained in a predetermined surrounding area of the arbitrary pixel, and calculates prediction probability for a pixel value of a pixel to be predicted on the basis of the disease at the pixel to be predicted and frequency distribution of the pixel value of the pixel to be predicted; an encoding unit (200) which encodes the pixel value of the pixel to be predicted on the basis of the prediction probability of the pixel value calculated by the pixel value prediction unit (190); and an output unit which outputs the code of the pixel to be predicted after the encoding by the encoding unit (200).
