<P>PROBLEM TO BE SOLVED: To achieve miniaturization of an image forming apparatus, improvement in assemblability, and reduction in costs, in the image forming apparatus having a plurality of recording heads and a cap member and a suction means as a maintenance recovery mechanism for the recording heads. <P>SOLUTION: A maintenance device 6A mainly comprises: a plurality of suction caps 10 as a cap member and a plurality of connection tubes 11 as a communication member, and a suction pump 13 as a suction means. An abutting part 10a, a storage 10b, a discharge hole 10e, and a plurality of connections 10d in communication with the discharge hole 10e and including discharge openings 10c extending in a substantially horizontal direction are integrally formed in each suction cap 10. Each suction cap 10 includes the plurality of connections 10d having the discharge openings 10c connecting respective adjacent suction caps 10 to discharge waste liquid.
