<P>PROBLEM TO BE SOLVED: To provide a delivery document causing no paper jam in the inside of a printer even in printing by a cut paper printer and also causing no such jam at paper discharge even in a production process. <P>SOLUTION: In a sheet-like delivery document 10, a slip paper 5 in which a delivery slip 1 and an attached slip 3 are sequentially provided is superposed on a rib portion 107, and is laminated via an adhesive layer 31, on the surface of a label substrate 21 of a label paper 20 comprising the label substrate 21, a tacky adhesive layer 33 and a release paper 25. Further, in a plurality of slip papers 5 in a delivery slip portion 101 perpendicular to a printer conveyance direction, perforations 53Y2 are applied on the release paper 25, and half cuts 43Y2 are applied except the release paper 25.
