Disclosed are a method, device and system for establishing a device-to-device connection, belonging to the technical field of communications. The method includes: a device-to-device (D2D) server acquiring register information about a first device which requires to perform D2D communication; performing pre-matching on the first device to find a second device according to the register information about the first device; respectively initiating a communication beginning request toward the first device and the second device, and triggering the first device and the second device to establish a D2D communication link between the first device and the second device through an MME and an evolved NodeB (eNB) to which they belong respectively. In the present invention, a D2D connection is established between two devices by triggering an eNB by an MME to which UE belongs, so that the establishment and use of a connection between two devices can be controlled by the network, thus realizing control, management, charging, etc. of D2D by the operators, and being able to control the interference to the ordinary UE by D2D.
