A pharmaceutical preparation comprises an anionic or cationic ester salt of a 21-hydroxy-20-keto-steroid in an aqueous solution having a pH of 5 to 8 and a physiologically tolerable non-ionic organic hydrotropic solubilizer which is an alkylamide, e.g. diethyl acetamide, alkyl-phenol formaldehyde-ethylene oxide condensate, e.g. tertiary octylphenol - formaldehyde - ethylene oxide condensate, lactic acid ester, polyethylene fatty acid ester, polyoxyethylene fatty acid glyceride, e.g. oxyethylated castor oil, propylene glycol, 1 - phenyl - 2,3 - dimethyl pyrazolone (5), sarcosine anhydride, N,N-dimethyldiketopiperazine, urethane, e.g. ethyl urethane, dextrin, dextran or water-soluble starch preparation. Specified steroids are malonates, succinates and substituted or p unsubstituted amino acid esters of 21 - hydroxy - 20 - keto steroids, e.g. pregnane - 21 - o1,20 - dione, D 4 - pregnane - 21 - o1 - 3 : 20 - dione, D 1:4 - pregnadiene - 17 : 21 - diol - 3 : 20 - dione; and their 11 - hydroxy-, 16 - hydroxy-, 11-keto-, 6a -methyl-, 16a -methyl- or 9-halogen derivatives. The salt of anionic esters may be the alkali metal or ammonium salts or the salts with alkylamines, hydroxy-alkylamines and hexamethylene tetramine. Salts of cationic esters may be those with inorganic acids, aliphatic carboxylic acids, acylated amino acids, dicarboxylic acids, hydroxycarboxylic acids and hydroxy dicarboxylic acids.
