<P>PROBLEM TO BE SOLVED: To easily manufacture a copy form capable of collecting information entered with an electronic pen. <P>SOLUTION: A pattern of a printing paper sheet is printed on a first form 11 and a second form 12 of a copy form 1 respectively, which are different in size. Then, the form 11 and the form 12 are piled up and bound with an adhesive or the like. Furthermore, a dot pattern is printed on an exposed surface of each form of the copy form 1 in ink which can be read with the electronic pen while the copy form 1 is fed to a sheet-fed press from the bound portion side.
