PURPOSE: A medical chart for a patient is provided to form a color table of patient's excretion and to display the state of the excretion. CONSTITUTION: A medical chart(100) for a patient comprises a record sheet(110), a hanging part(120), a holder(130), and a color chart(140). The hanging part is formed at the upper end portion of the record sheet and is fixed at a hospital bed. The holder is formed at the upper portion of the record sheet.
