In order to independently control the loss and phase difference of light passing through an optical waveguide device by means of a simple configuration, this optical waveguide device is provided with: an optical waveguide provided with two electrodes that respectively impose two signals; and a control means that controls the two signals in a manner so that the loss change and phase change arising at the optical waveguide by means of the two signals become predetermined values.
