Provided are an IPTV fault location method, device and system. The method includes: performing synchronous evaluation on the monitored IPTV video streams according to a synchronous message to obtain a plurality of corresponding local upstream evaluation results; receiving an evaluation result report message sent by a downstream monitoring point, with the evaluation result report message containing downstream evaluation results, evaluation result identifiers and synchronous message information; searching for upstream evaluation results corresponding to the downstream evaluation results according to the synchronous message information and the evaluation result identifiers in the evaluation result report message sent by the downstream monitoring point; and comparing the downstream evaluation results with the upstream evaluation results, and performing fault location according to the comparison result. In the present invention, the location of IPTV faults is realized, reducing manpower and material expenditure, and improving the fault location efficiency.
