This optical semiconductor device includes: a flexible substrate that has, on a surface, multiple photoelectric conversion elements and element electrodes that are respectively connected to the multiple photoelectric conversion elements; a positioning means for placing at least one of the photoelectric conversion elements at the use position by moving the flexible substrate in the array direction of the multiple photoelectric conversion elements; and substrate guides for guiding the movement of the flexible substrate by having the flexible substrate remain in contact in a slidable manner. The substrate guides have conductive parts that come into contact with the element electrodes at the use position.
