<P>PROBLEM TO BE SOLVED: To provide a template for ensuring and facilitating an operation for deforming and fixing a bone by fastening a screw for medical treatment that requires skill, by using the specific template. <P>SOLUTION: The template is a combination of: a template body A integrally comprising a fitting part 1 having an inner surface joined and fitted to the three-dimensional shape surface of a bone part, and cylindrical spacer tube guide cylinder parts 2 projected in a direction coaxial with the insert direction of the fitting part 1 and the screw for medical treatment; and cylindrical spacer tubes B sliding in internal contact with the cylindrical spacer tube guide cylinder parts 2.
