FIELD: medicine. ^ SUBSTANCE: 4-5 cm long cut of mucosa is performed in projection of anterior edge of lower jaw branch with transition on transitional fold. Coronoid process is captured with Billroth clip, which is used to manipulate the lower jaw. Hemostasis is performed. Endoscope is introduced into wound. Fracture line is viewed. Displaced condylar process is removed from wound, microplate is fixed to it outside wound. After that, process together with attached to it microplate is introduced into wound in such a way that head gets exactly into joint fossa, and fixed to branch with two screws in correct anatomical position through preliminarily made cut in parotid-masticatory region in fracture projection. Accuracy of alignment is checked by means of endoscope. Wound is processed with antiseptic and sutured layer-by-layer. Immobilisation is enforced by temporary inter-maxillary splinting on alveolar screws with rubber rods. ^ EFFECT: method makes it possible to realise osteosynthesis of displaced condylar process under skull base with miniplates, prevent development of post-operative scars on skin, eliminated re-operation aimed at removal of metallic constructions due to open reposition and metal osteosynthesis of mandible with application of endoscopic equipment. ^ 2 ex |
