Systems and methods for remote endarterectomy are disclosed. In an embodiment, a device (100) for remote endarterectomy includes an elongated member (102), having a proximal end (103), a distal end (104), and a longitudinal axis therebetween; an endarterectomy unit (105) at the distal end (104) of the elongated member (102) formed by a first member (303) engaged to a second member (301) in a substantial secure alignment with the second member (303), wherein the endarterectomy unit (105) has a circumferential enclosure (107) configured to separate a plaque core from a blood vessel and an open region (109) surrounded by the enclosure (107) and configured to receive the separated plaque core therethrough; and an actuator (113) coupled to the proximal end (103) of the elongated member (102) for translating the first member (303) relative to the second member (301) across the open space (109) to transect the plaque core.
