<P>PROBLEM TO BE SOLVED: To provide a repeatedly usable form for forming a grout injection/discharge hole, which is easily handleable and excellent in workability and which can form a grout injection (discharge) hole in a predetermined shape. <P>SOLUTION: A form for forming a grout injection/discharge hole communicating with the inside of a sleeve joint of a precast concrete structure comprises a pipe body that is expanded and contracted, and a core material for a mold, which is inserted into the pipe body and which has an outside diameter larger than an inside diameter of the pipe body. Preferably, either an inner peripheral surface of the pipe body or an outer peripheral surface of the core material for a mold is formed in an irregular shape.
