FIELD: medicine. ^ SUBSTANCE: invention relates to surgery and can be applied for treatment of duodenal fistula in case of failure of stump after stomach resection and gastrectomy. At the first stage fistula is excised and resection of duodenum stump is performed, drainage of ductal system of pancreas and common bile duct is carried out, removal of pancreatic juice and bile outside and their recuperation into digestive system is performed by means of nasointestinal probing. At the second stage medial resection of pancreas is carried out, separate drainage of ductal system of proximal and distal stump of pancreas is performed, single-row choledochoduodenoanastomosis of "end-to-end" type is formed, two pancreaticoenteroanastomoses of loop-end structure are formed with external removal of pancreatic secretion from zone of pancreaticoenteroanastomoses for the period of anastomoses healing. ^ EFFECT: method makes it possible to prevent dehydration and electrolyte and protein imbalance, reduce risk of purulent-inflammatory complications. ^ 1 ex, 4 dwg |
