<P>PROBLEM TO BE SOLVED: To provide an input device that includes an input operation part for receiving input operation and swingably provided to a device body via a tilt mechanism, and a slide device that is stored in the input operation part, and prevents damage to the slide device when the slide device is protruded from a storage position, as well as an image forming apparatus including the input device. <P>SOLUTION: An input device (image reading device) 2 includes: an input operation part 6 that receives input operation and is swingably provided to a device body 2a via a tilt mechanism 10; and a slide device 20 that is stored in a storage position of the input operation part 6, and slides to be protruded from the storage position.
