<P>PROBLEM TO BE SOLVED: To easily and simply form a hologram on the surface of a packaging material for a paper container. <P>SOLUTION: A paper base material 14A and a hologram sheet 16 are fed to a space between a nip roller 32 and a cooling roll 33, and a molten resin 12A for hologram formation is fed to a space between the paper base material 14A and the hologram sheet 16. A hologram 12a is formed by the hologram sheet 16 on the surface of the molten resin 12A for hologram formation in a space between the nip roll 32 and the cooling roll 33. In this way, a sheet material 30 with a three layer structure of the paper base material 14A, a hologram-formed resin layer 12 having the hologram 12a and the hologram sheet 16 is formed.
