<P>PROBLEM TO BE SOLVED: To comprehensively collect replies associated with respective types of activities that are performed by object persons with actual conditions of the activities of the object persons previously grasped and classified, in a question paper survey system. <P>SOLUTION: An activity information acquisition part 10 acquires activity information of an object person. An activity distribution information storage part 21 stores distribution information in which the activity information of the object person is classified by cluster classification. A reply acquisition condition storage part 22 stores a reply acquisition condition of the object person in accordance with a cluster of the distribution information. A reply request determination part 3 determines whether to make a reply request to the object person on the basis of the activity information, the distribution information, and the reply acquisition condition. When it is determined to make a reply request, a reply request notification part 14 notifies the object person of the reply request and a question item provision part 15 provides question items to the object person.
