<P>PROBLEM TO BE SOLVED: To easily create a business form having items corresponding to various tests for chemical material toxicity tests. <P>SOLUTION: Use frequency update means 62 updates use frequency of respective business form format tables used for outputs of business forms as use frequency of respective test types. When business forms 47 and 48 are output, business form format table output means 63 acquires a business form format table having the highest use frequency for respective test types as the highest use frequency business form format table, and outputs it to business form output means 61. The business form output means 61 creates a template having a plurality of output items and data entry areas for symptom change data corresponding to the output items by using the highest use frequency business form format table. Data insertion means 66 inserts the symptom change data associated with a living body in the data entry areas by using the template and creates the business forms 47 and 48.
